Directions
When you arrive for your appointment, you can enter the building on Belmont. This door is locked, so you will need to enter a code (which you may get from the intake clinician). When you enter the building, you will see a door on your left and a sign for Lakeview Psychology Group.
New Clients
To provide Lakeview Psychology Group with the basic registration information needed before we schedule your first appointment, please email or call (email preferred) and speak to our intake clinician. When you contact our intake clinician, please have all you health insurance information available. Once your intake information has been processed, the therapist working with your family will contact you within 24 hours to set up your first visit.
For your first appointment, please arrive 15 minutes early. You will find your paperwork on the file holder outside the office door. Some of the forms may be downloaded from this website prior to your appointment and you may save time by filling out these forms before your session. Please bring a photo ID, your insurance card, a credit card to place on file, and any paperwork you have completed. You may be seated in the waiting area until your appointment time.

Insurance and Payments
LPG therapists are in-network providers for Illinois Blue Cross/Blue Shield PPO. If another carrier insures you, your health insurance may cover a portion of our services as Out of Network Providers. Lakeview Psychology Group is happy to provide you with an invoice that you can submit to your insurance company for partial or full reimbursement, and to help walk you through this process should you need our assistance.
Lakeview Psychology Group accepts cash, venmo, chasepay, or credit card (Visa, Master Card, Discover). Payment is due at the time of service.
